First Visit
Initial appointments are essential because they help us get information about your background and current issues. The first appointment includes a consultation where one of our professionals explains your diagnosis and treatment options. If a minor surgery is needed, it can be completed in the same day. However, more complex treatments might necessitate further evaluation and a second appointment.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and x-rays.
- A thorough list of any medications you are currently taking.
- Completed medical and dental insurance forms to save time during the consultation that can be spent discussing your diagnosis and treatment options.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian by law.
Please remember that a preoperative consultation and physical examination is mandatory if you are a patient who needs IV anesthesia during surgery. You cannot have anything to eat or drink for eight hours prior, and an adult needs to drive you home from the office.
If you have a medical condition that could affect surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, etc.) please inform the office beforehand. Please alert us if you are taking any medications as well, as they can affect the anesthetic.
X-Rays
If you have previous x-rays from a dentist or physician, you can request that they be forwarded to our office. If this cannot be done, please bring physical copies with you to your appointment. Additional x-rays can be completed at our facility if needed.
